The Bethany College's athletic training department is committed to providing comprehensive, high-quality medical services to every student-athlete. On-site medical services provided include, but are not limited to: prevention, evaluation, and treatment of injuries and illnesses; rehabilitation services; pre-participation exams; nutritional counseling; psychological counseling; and drug education and counseling. Our goal is to return our athletes on their playing field as quickly and safely as possible.

All athletics paperwork must be completed/uploaded to Healthy Roster. Below you will find instructions on this process. Please keep in mind that these forms below are not to be confused with the documentation required to also be completed by all students for Bethany College Student Health Services. Those forms can be completed by contacting wellnesscenter@bethanywv.edu.

If you have ever been diagnosed or are currently under treatment for any of the following conditions, you MUST provide documentation (including a list of medications) from your physician that you are fully cleared to participate (no restrictions) in athletic activities:

• Surgery in the past calendar year
• Any cardiac conditions/murmurs
• Diabetes
• ADD/ADHD
